Traditional skin rejuvenation treatments include topical creams, dermal abrasion, chemical peels and ablative lasers such as the CO? and ER: Yag ablative lasers. These are effective in reducing wrinkles from aging or photo damage, but result in side effects that are painful and require significant recovery time. While remodelling of the dermis is required for effective skin resurfacing, these ablative treatments also destroy the epidermis to some extent. Several layers of the skin are removed during treatment, and the affected areas are often left with open wounds. Common side effects include erythema, scarring, pain and infection and further sun damage if clients do not adhere strictly to aftercare instructions. Consequently, these treatments can require several weeks of patient recovery time.

The introduction of nonablative (non-invasive treatment with GentlemaxTM) lasers has made it possible for patients to seek treatment with little interruption to their work or social schedules. Nonablative lasers selectively target the dermis, while protecting the epidermis from injury. This selective remodelling of the dermis is responsible for the improvement in skin appearance with minimal post procedural side effects.

Nonablative Laser treatments are able to provide a more immediate result than most anti-aging creams and facials that only work and manage to penetrate the skins surface. They have the advantage of working at cellular level to stimulate the body’s own collagen and elastin formation and restore the skins function. However better results have been achieved in cases where laser is used in conjunction with other technologies such as Botox treatments.

How does laser rejuvenation and wrinkle reduction work?

What does a typical treatment include?

The area to be treated is cleansed, removing all makeup and creams. Laser is then administered, focusing on those areas of concern. A collagen enhancing mask is then applied to the entire area and left to absorb for up to 15 minutes. Finally the mask is removed and aloe vera gel with Solarcaine and menthol is applied followed by moisturizer and sunscreen. The results become more obvious several days after treatment!

How many treatments are required?

Several treatments done over the course of a few months will produce significant and lasting results. We do recommend maintenance treatments every few months thereafter which are optional, if you want to retain your new youthful appearance. This will be discussed at your initial consultation as there are many factors that will influence the desired outcome, such as age, genetics, diet, exercise, condition of skin and your own beauty routine and whether you smoke, etc.
But that’s not all, as collagen remodeling is a slow process, you will continue to see improvement up to 6 months after your last treatment

What do treatments feel like?

There is a little discomfort involved with each pulse when you will feel a brief heating sensation followed by a spray of coolant (DCD) that minimizes any tenderness. Most clients tolerate the treatment well without the use of anesthetizing creams

Can treatments be performed over Botox and fillers?

Treatments can be safely performed over Botox and there is no clinical evidence that the filler breaks down any sooner after laser treatments. Please note that laser treatment should only be performed 2 weeks prior to, or after Botox or filler treatments.

Are there contra-indications for treatment?

Recently tanned skin should not be treated. Both natural sunlight and tanning booths should be avoided at least 3 weeks prior to treatment.

Why is there a need for multiple and maintenance rejuvenation and wrinkle reduction treatments?

Results vary with each case. The number of treatments each client requires is determined by the condition of your skin, and may require anywhere from 2 - 6 treatments. As we age, our bodies reduce the production of collagen. At 65 years of age most women have stopped producing collagen. Rejuvenation treatments result in the laser stimulating the body into producing or reorganizing its own collagen.

Are there any side effects to treatment?

Rejuvenation treatments should cause only minimal discomfort. We expect clients to have mild erythema (swelling) and redness of the areas that are treated which usually lasts between 15 minutes to a few of hours.
Clients are advised to apply aloe vera gel and moisturizer at least twice a day followed by SPF 30+ sunscreen during the day. Treated skin may feel tight and dry and require extra moisturizing.
No downtime or bruising is experienced and clients should resume their normal activities immediately.
A list of pre and post treatment care procedures will be discussed with you in detail at your initial consultation and given to you in writing.

How long do results last?

Just like the aging process itself, this varies from person to person. However, if you take steps to protect and preserve your freshly rejuvenated skin by using sunscreen, facial creams and effective moisturisers, the results will last longer.
It is important to note that UVA and UVB rays in sunlight produce enzymes that break down the skin’s supportive structure and so prevent formation of new collagen.

Understanding the aging process:
Collagen, elastin (both proteins) and hyaluronic acid (which gives skin volume) decreases as skin ages. There is a decline in the number of collagen fibres and an increase in thickness of these fibres due to cross linking. These changes result in a decreased ability for the skin to retain water and the rearrangement of fibres contributing to wrinkle formation. Aged skin is usually dry, dehydrated and lacks elasticity commonly as a result of harmful UVA and UVB rays in sunlight that produce enzymes that break down the skin’s supportive structure and so prevent formation of new collagen. There are three phases which comprise aged skin: development phase, mature phase and senescence phase.

There are two main processes of skin ageing: intrinsic and extrinsic. Intrinsic ageing is involuntary and reflects the genetic background of the person and is therefore inevitable, whereas extrinsic ageing is caused by external factors such as sun exposure, poor nutrition, smoking and alcohol among others which we can control. Essentially, extrinsic ageing is the ageing we experience due to environmental factors with a major factor being the sun. There are numerous signs of photo-aging that are essentially related to skin colour and more importantly a person’s exposure to sun. Ageing is apparent in one’s skin through the manifestations of telangiectasia, skin wrinkling, leather-like dry skin, lentigenosis and hyperpigmentation.  Collagen in the dermis becomes calcified, with a loss of bonding strength between the epidermis and the dermis leading to deep lines, furrows and wrinkling of the superficial skin. There are two types of wrinkles, Dynamic wrinkles which occur when we form facial expressions and Static wrinkles which are present when the face is at rest.

Overall, exposure to ultraviolet (UVA or UVB) radiation from sunlight accounts for about 90% of the symptoms of premature skin ageing, and most of these effects occur by age 20. Effectively protecting the skin from sun damage is one of the most important ways to reduce wrinkles and prevent skin cancer.
